Ava Richardson The 2023 Chronic Disease Management Summit will focus on prevention, early identification and management of diabetes, hypertension, high cholesterol, and arthritis. This event emphasizes a team-based approach to effective management, recognizing theimportance of collaboration among healthcare professionals. Key topics will include: an overview of prevalence in Kansas; impact of social determinates of health on development of risk factors; and current approaches to diet and exercise.

Resource Center for Chronic Disease Management
This virtual exhibit hall provides participants with an opportunity to visit one-on-one with organizational representatives in a relaxed environment. The information available includes patient educational materials, tips on utilizing equipment, nutrition, and exercise. Below you will find a link to join the virtual exhibit hall as well as handout materials available from each of the exhibitors.

After you log on to Zoom, if you discover you need to change your name to display your first and last name, click on the 'Participants' tab at the bottom of your screen where you see other meeting controls. K-State Research and Extension
Representatives:
Sharolyn Jackson (Northeast) and Ashley Svaty (Northwest) are Family and Consumer Sciences Regional Specialists for Kansas State Research and Extension (KSRE). They provide leadership and program coordination for several chronic disease prevention programs centered around increasing physical activity. Their work is focused on research and evidence-based health and wellness program development and implementation; concentrating on policy, systems and environmental change; and educational programming to motivate individuals to eat more healthfully, be more active, and manage stress effectively, to promote physical and mental wellbeing.
Christina Holmes (Southeast) is the Kansas Family and Consumer Sciences Regional Specialist for Kansas State Research and Extension and is also a Registered Dietitian. She provides leadership and program coordination for Kansas State Research and Extension in the Dining with Diabetes and ServSafe programs. Her work is focused on research and evidence-based health and wellness program development and implementation; concentrating on policy, systems and environmental change, and on educational programming that motivates individuals to eat healthier and be more physically active.
KSRE led programs that will be highlighted include:
- Walk Kansas -- An 8-week KSRE health initiative that supports adoption of healthy lifestyle habits, such as: being more active with support from friends and family, making healthier nutrition/beverage choices, and practicing helpful techniques to reduce stress. Walk Kansas is primarily team-based with options for individual participation.
- Stay Strong Stay Healthy (SSSH) -- An evidence-based 8-week program for older adults that meets the recommendation for healthy muscle strength. The program's goal is to improve health and quality of life. Sessions are led by local Extension agents that are trained/certified to lead these classes. Each hour-long session (total of 16) includes warm-up activities, strengthening exercises with or without weights, and cool-down stretches.
- Walk With Ease -- A community-based physical activity and self-management education program from the Arthritis Foundation. While walking is the central activity, WWE is a multi-component program that also includes health education, stretching and strengthening exercises, and motivational strategies. The program includes a guidebook and a walking schedule. Sessions are led by Extension agents and SNAP-Ed Nutrition Educators who are trained/certified instructors.

KDHE's Community Health Promotion team works to improve Kansans’ health and wellness by creating tobacco free environments and promoting tobacco prevention and treatment, healthy food access and active living. Tobacco Use Prevention Program Tobacco is the leading cause of preventable deaths in the nation. The Kansas Tobacco Use Prevention Program (TUPP) provides resources and assistance to state and local partners for development, enhancement and evaluation of state and local initiatives to prevent death and disease from tobacco use and secondhand smoke exposure. TUPP focuses on four priority areas: 1) Preventing the initiation of tobacco use among young people, 2) Promoting quitting among tobacco users of all ages, 3) Eliminating nonsmokers' exposure to secondhand smoke, and 4) Identifying and eliminating disparities related to tobacco use and its effects among different population groups. In addition, TUPP manages the statewide Kansas Tobacco Quitline, a free resource that provides services and support to Kansans trying to quit tobacco products. The Quitline offers 24/7 counseling services and specialized support to youth, those with behavioral health conditions, pregnant women and American Indians. TUPP is also home to Resist, a youth led tobacco prevention initiative that empowers youth to stand up against the tobacco industry and prevent youth tobacco initiation.

The Landon Center on Aging, part of the University of Kansas, is an interdisciplinary hub for education, research, health care and community engagement for older adults.
It includes an optimal location for community participants to engage in a variety of outreach programs from wellness and exercise to elder law clinics and weekly educational presentations, a Geriatric Medicine Clinic providing both primary care and consultative services, facilities for researchers to study important aging-related issues such as prevention and treatment of disability, patterns of healthcare delivery, and cognitive function, as well as coursework for the next generation of healthcare providers to learn from faculty about the care of older adults in state-of-the-art classrooms

Lindsey Yeager - a Registered Dietitian, Certified Diabetes Care and Education Specialist working on the clinical side of Novo Nordisk. She supports 4 large health systems in the state of Kansas to identify strategies for improving the health of patients with diabetes.
She will be sharing the 2023 ADA guideline updates, as well as Novo Nordisk’s health coach program information.
Patient Experience in Rural Kansas (PERK) Project

The Patient Experience in Rural Kansas (PERK) Project is a partnership of the United Methodist Health Ministry Fund (Health Fund) and the Kansas Frontier Community Health Improvement Network (KFCHIN). The Health Fund is a nonprofit organization supporting innovative, collaborative initiatives for the health and wholeness of all Kansans. KFCHIN unites representatives of healthcare facilities from several western Kansas "frontier" counties, as well as state associations and philanthropic groups. As a grantee under HRSA's Small Health Care Provider Quality Improvement Program, the Health Fund engaged KFCHIN in August 2019 to assess patient experience and available resources, develop and implement strategies, and share best practices to improve quality of life and clinical outcomes for rural patients living with type 2 diabetes. As the PERK Project nears its end later this month, coordination consultant Jessica Ardery looks forward to sharing some key takeaways, most valuable resources and a brief virtual tour of the updated PERK Central 2.0 Resource Hub, a grant work product available to other diabetes care stakeholders for purposes of networking and linkage to cost-free patient and professional resources. Course Faculty
Mary Hastert, RD, MS
Mary Hastert is a registered dietitian and Ph.D. candidate at the University of Kansas Medical Center. Her research focuses on the development and implementation of lifestyle interventions in adults with chronic diseases, specifically obesity and metabolic conditions. Her role in the KUMC’s Department of Internal Medicine aims to integrate outpatient weight management care with research outcomes to evaluate the effectiveness of obesity interventions delivered in a clinical setting. Her long-term goal is to develop multi-component lifestyle interventions that improve clinical guidelines for obesity care in adults with cardiometabolic conditions.
Shannon Metz, MPH
Shannon Metz is a Senior Chronic Disease Epidemiologist at the Kansas Department of Health and Environment. She has been with KDHE for over 5 years. She currently supports the Community-Clinical Linkages Section, which houses arthritis, cardiovascular disease, and diabetes.
Stacie Stoltz, MSW
Stacie Stoltz received a Bachelor of Science in psychology from Kansas State University and a Master of Social Work from the University of Kansas. She is a licensed specialist clinical social worker and since 2016 has been clinical manage-adult behavioral and mental health at the University of Kansas Health System-Strawberry Hill Campus.

Sarah Aptilon, PhD
Sarah Aptilon is an Associate Professor in the Department of Philosophy and Religion at Johnson County Community College, where she teaches courses on world religions, Asian religions, and Japanese culture. Sarah teaches meditation classes at Turning Point, a subsidiary of The University of Kansas Health System. She lived and practiced in a Buddhist monastery in Kyoto for seven years, and has taught meditation in various settings since 1996. Over the past seventeen years Sarah has conducted hundreds of cross-cultural training programs for global companies throughout the U.S. and Mexico. She earned a Ph.D. in Buddhist Studies from Stanford University and a B.A. in East Asian Studies from Yale University.
Crystal Burkhardt, PharmD, MBA, BCGP, FCCP
Crystal Burkhardt, PharmD, MBA, BCGP, FCCP is a Clinical Professor at the University of Kansas School of Pharmacy, and a geriatric clinical pharmacist at the University of Kansas Medical Center’s Primary Care & Geriatric Clinics. She has developed multiple ambulatory pharmacy practices throughout her career, and currently provides consultative services for ambulatory geriatric patients within KUMC’s Primary Care & Geriatric Clinics. Her teaching responsibilities include required and elective didactic geriatric pharmacotherapy instruction to pharmacy and medical students. She precepts pharmacy and numerous other interprofessional learners during their geriatric clinical rotations. Her research interests include interprofessional teaching and practice models centering on the care of the geriatric patient.
Lianna Fry, BS
Lianna Fry is the Greater Wichita YMCA's Senior Program Director of Community Health. Lianna has been with the Greater Wichita YMCA since 2007 after graduating from Kansas State University with a Bachelors Degree in Kinesiology. Her current role provides oversight to the the YMCA's evidence-based health intervention programs, referral partnerships, and corporate partnerships for employee wellbeing. Lianna coordinates community partnerships, and new program development and implementation.
Lianna's certifications include ACSM Exercise Physiologist, Wellcoaches Health and Wellbeing Coach, ACLM Lifestyle Medicine Coach, and YMCA Diabetes Prevention Program Lifestyle Coach.

Anthony Jackson, MD
Dr. Anthony Jackson is a board certified Internal Medicine physician. He completed his medical school, residency training and currently, a Geriatric Medicine fellowship at the University of Kansas Medical Center. He has worked at a variety of clinical sites including rural Southeast Kansas, Kansas City, Leavenworth, and Topeka. Later this year he will be starting a new chapter in Kansas City, taking on a primary care/geriatrics practice and medical directorship of a large nursing facility. He feels a responsibility to care for older adults and enjoys geriatrics because it allows a unique opportunity to focus on the whole patient, not just their diagnosis.
Amanda Johnston, PharmD
Amanda Johnston is a clinical pharmacist who works in the Family Medicine Clinic at the University of Kansas Health System. She works with patients to help control their chronic disease states by making medication adjustments and providing medication and device education.
Justin Moore, MD, FACP
Justin does consulting work at the clinical-public health intersection with Double Arrow Metabolism. It's in this role that he serves as Medical Director for the Kansas Business Group on Health. He also serves as an endocrinology consultant with RubiconMD, as a medical consultant for Change Healthcare and as Director of Nutrition and Metabolism Education for the University of Kansas School of Medicine's Department of Internal Medicine in Wichita. He believes chronic diseases should fall at the top of the patient's problem list at any given visit, not at the bottom. He believes that health is hard to define and harder to measure, but that it's easier to achieve than people think. He believes true health saves money.
Hannah Poppen, PharmD, BCACP
Hannah Poppen is a board-certified ambulatory care pharmacist at the University of Kansas Health System in the Family Medicine clinic. She received her doctorate in pharmacy at South Dakota State University. Dr. Poppen completed a PGY1 pharmacy residency at the Sioux Falls VA Medical Center, and a PGY2 in ambulatory care at Mayo Clinic Health System. She works with patients referred by their primary care provider for comprehensive medication evaluation and chronic disease state management, including diabetes, hypertension, dyslipidemia, obesity, and polypharmacy.
